Hello Lafayette families! As we get ready to wind down for summer, it is that time to ensure you are submitting your health forms for returning students! Every student needs a Universal Health Certificate and an Oral Health Assessment (i.e. health and dental forms!). These forms expire a year from the date of exam and must be resubmitted every year! Students will also need to be up to date on immunizations as DC Health plans to enforce “No Shots, No School” next year. Please submit all forms to . If you submitted them to the main office already, I should hopefully have them soon! Please submit updated medication forms directly to me via the health forms Gmail account or .
I will be here working on forms throughout the summer, so the earlier you can submit them, the better!
